We are actively seeking an MDS Coordinator (Lead) for Victoria Crossing Rehabilitation Center. In this role, you will be responsible for overseeing the MDS process and providing support to the care planning team.

Responsibilities:

• Evaluate and ensure the quality of care for long-term care residents.
• Conduct initial interviews and assess new residents.
• Develop comprehensive care plans with both short-term and long-term improvement goals.
• Facilitate meetings with multidisciplinary treatment teams, caretakers, and families.
• Collaborate with community specialists to achieve care plan objectives.
• Attend Nursing management team meetings and interact effectively with all levels of Nursing staff.
• Conduct in-service education sessions on MDS, RAPS, and Triggers.
• Adhere to the MDS/Care Planning schedule for non-Medicare residents.

Qualifications:

• Valid Florida RN license
• Minimum of 2 years of experience in a Skilled Nursing Facility
• Knowledge of MDS, Medicare, and the care-planning process
